This service is aimed at supporting individuals to either regain or relearn lost skills and abilities or to help individuals gain new skills, whilst living at home. This enables them to achieve a higher quality of life, personal goals and live as independently as possible.
The service also aims to help individuals live in the way they may have lived before their condition deteriorated and aims to reduce or prolong the need for care where this is achievable. In this respect our staff are trained in supporting individuals to achieve and retain skill sets, we refer to this type of knowledge as ‘enablement competencies.’An example of how this service supports individuals, is where an individual has been discharged from hospital following a hip operation. The six weeks following the operation tend to be a very difficult time, with a lot of support being required to ensure that recovery progresses in the right direction.
